How Referrals Work
1. Submit a Referral
Complete our referral form with information about your client and their housing needs.
2. Eligibility Review
Our team reviews the referral to determine eligibility and identify an appropriate housing option.
3. Placement Coordination
When an appropriate opening is available, we coordinate the required documentation, funding, and placement details with the referring professional.
4. Move-In
Once requirements are completed, we coordinate move-in and provide the information needed for arrival.

How We Use Our Homes
Properties leased by Sable Haven are used to provide shared independent living housing for adults participating in our housing program. We seek property owners who are comfortable with this use and willing to establish lease terms that permit Sable Haven to provide housing to program participants.
How Property Partnerships Work

Our team reviews the property, location, layout, availability, and other details to determine whether it may meet our housing needs.
3. Discuss Lease Terms
If the property appears to be a good fit, we discuss the proposed lease structure, permitted use, occupancy, and other important terms with the property owner.
4. Establish the Partnership
Once terms are agreed upon and the lease is completed, Sable Haven coordinates the property setup for use within our housing program.
